You can request a certified true copy of a death certificate for any death that was registered in Madridejos. You need this for insurance claims, estate settlement, pension transfer, SSS/GSIS claims, and other legal purposes. The fee is ₱150.00 per copy.

Who May Avail

Immediate family members of the deceased (spouse, children, parents, siblings) or an authorized representative with a signed authorization letter.

Requirements

1 valid ID of the person requesting (government-issued)
If you are requesting on behalf of someone else: signed Authorization Letter plus a photocopy of the authorizing person's valid ID
Full name of the deceased and approximate date of death (to help the staff locate the record)
